OT Intake Form

If you are a case manager or other health professional and would like to make a referral to our Home Accessibility Team, please fill out the submission form below. We provide home safety assessments performed by CAPS (certified aging-in-place specialist) Occupational Therapists and make individualized recommendations for home modifications and equipment. We then obtain the equipment if able with grant funding and provide training in home on how to safely utilize that equipment.
